ABSTRACT

Abstract Introduction: Highly active antiretroviral therapy (HAART) transformed HIV from a fatal disease to a chronic one, but it has adverse effects, such as the lipodystrophy syndrome, characterized by morphological and metabolic changes, such as reduced bone mineral density (BMD), potentiating morbidities and mortality. Strength training (ST) aims to increase BMD, due to the osteogenic effect. Objective: To verify the impact of strength training on BMD in people with HIV. Methods: This is a quasi-experimental study, which included 40 people with a mean age of 50 ± 6 years, separated into trained group (TG, n = 20) and control group (CG, n = 20), with reduction in BMD, HIV-positive, using HAART and without exercising. BMD was assessed by DEXA in the lumbar spine, femoral neck and distal 1/3 of the radius, before and after 12 weeks, with the GT submitted to 36 ST and the CG without physical training in the DEXA evaluation in the same time interval. Results: TG had a significant increase with great effect on BMD in all segments: lumbar spine (p = 0.001; ES: 1.87), femoral neck (p = 0.003; ES: 2.20) and 1/3 distal of the radius (p = 0.001; ES: 1.81). Meanwhile, CG group showed a significant reduction with great effect on the femoral neck (p = 0.020; ES: 2.56) and 1/3 distal of the radius (p = 0.015; ES: 2.93), while the lumbar spine showed a great effect to reduce BMD (p = 0.293; ES: 1.78). Conclusion: ST can be used as a therapeutic resource to increase BMD in people with HIV, contributing to the advancement in the search for non-drug therapeutic practices.


Resumo Introdução: A terapia antirretroviral altamente ativa (HAART) transformou o HIV em uma doença crônica, apresentando efeitos adversos como a síndrome da lipodistrofia, caracterizada por alterações morfológicas e metabólicas, como redução da densidade mineral óssea (DMO), potencializando morbidades e mortalidades. O treinamento de força (TF) tem como proposta aumentar a DMO, devido ao efeito osteogênico. Objetivo: Verificar o impacto do TF na DMO em pessoas com HIV. Métodos: Trata-se de um estudo quase-experimental que incluiu 40 pessoas com idade média de 50 ± 6 anos, separadas em grupo treinado (GT, n = 20) e grupo controle (GC, n = 20), com redução na DMO, HIV positivo, usando HAART e sem praticar exercícios físicos. A DMO foi avaliada pelo DEXA na coluna lombar, colo do fêmur e 1/3 distal do rádio, antes e após 12 semanas, com o GT submetido a 36 sessões de TF e o GC sem exercício durante o mesmo período. Resultados: O GT teve aumento significante com grande efeito em todos os segmentos: coluna lombar (p = 0,001; ES: 1,87), colo do fêmur (p = 0,003; ES: 2,20) e 1/3 distal do rádio (p = 0,001; ES: 1,81), enquanto o GC apresentou redução significante com grande efeito no colo do fêmur (p = 0,020; ES: 2,56), 1/3 distal do rádio (p = 0,015; ES: 2,93) e apenas grande efeito na coluna lombar (p = 0,293; ES: 1,78). Conclusão: O TF pode ser utilizado como recurso terapêutico para aumentar a DMO em pessoas com HIV, contribuindo para o avanço nas buscas de práticas terapêuticas não medicamentosas.

ABSTRACT

Abstract Background Patient self-report is the most common diagnostic tool in the literature to detect HIV/HAART-associated lipodystrophy. However, data on the association of cardiovascular risk factors with HIV/HAART-associated lipodystrophy assessed by self-report are still missing. Objectives To determine the prevalence of self-reported HIV/HAART-associated lipodystrophy and to identify independent associations between traditional modifiable cardiovascular risk factors and self-reported lipodystrophy. Methods We conducted a retrospective observational study at an outpatient infectious disease clinic in the Central-West of Brazil to identify the association between traditional modifiable cardiovascular risk factors and self-reported lipodystrophy. Sedentary lifestyle, smoking status, family history of cardiovascular disease, hypertension, diabetes, dyslipidemia, increased waist circumference and overweight were the cardiovascular risk factors assessed. Self-reported HIV/HART-associated lipodystrophy was categorized as: mild (noticeable by patients' close inspection), moderate (easily noticeable by patient and physician) or severe (readily noticeable by a casual observer). Prevalence ratio (PR) and 95% confidence interval (CI95%) were calculated. Multivariate Poisson's regression was used to analyze factors associated to HIV/HAART-associated lipodystrophy assessed by self-report considering a significance level of 5%. Results A total of 183 patients were included, with a mean age of 39.3±10.9 years. Most of the sample were male (77.6%), non-white (50.8%) and single (53.0%). The overall prevalence of HIV/HAART-associated lipodystrophy was 52.5% (95% CI 44.96 - 59.88). Severe lipodystrophy was observed in more than half patients (55.2%). No traditional modifiable cardiovascular risk factor was independently associated with lipodystrophy. Female sex (PR 1.49; 95% CI 1.15 - 1.95; p =0.003), time of HIV infection diagnosis of 1-3 years (PR 1.83; 95% CI 1.09 - 3.08; p =0.002) and a positive family history of CVD (PR 1.62; 95% CI 1.11 - 2.36; p <0.001) were independently associated with lipodystrophy. Conclusion HIV/HAART-associated lipodystrophy assessed by patient self-report was not associated with traditional modifiable cardiovascular risk factors. Resumo O uso de antirretroviral aumentou a sobrevida dos portadores do HIV, porém pode acarretar efeitos colaterais, como a síndrome lipodistrófica. O objetivo deste artigo é identificar a frequência da síndrome lipodistrófica e seus fatores associados em pacientes portadores do HIV em uso de terapia antiretroviral. Estudo transversal com pacientes acompanhados ambulatorialmente. A síndrome foi avaliada pela associação de dois parâmetros: emagrecimento periférico através da escala de gravidade de lipodistrofia e acúmulo de gordura central, mensurado pela relação cintura quadril. Para identificar as variáveis associadas foi realizada a análise de Regressão de Poisson. Dos 104 pacientes avaliados, 27,9% apresentaram a síndrome. Após ajuste, ser do sexo feminino (RPajustada = 2,16 IC95%1,43-3,39), ter excesso de peso (RPajustada = 2,23 IC95%1,35-2,65) e um maior tempo de uso dos antirretrovirais (RPajustada = 1,64 IC95%1,16-2,78) permaneceram positivamente associados à síndrome. Por outro lado, foi observada uma associação negativa com a contagem de CD4 £ 350 (RPajustada = 0,39 IC95%0,10-0,97). A alta prevalência da síndrome e sua associação com grupos específicos reforçam a necessidade do adequado acompanhamento e identificação precoce como forma de intervir nos fatores modificáveis.


Abstract The use of antiretroviral drugs has increased the survival of HIV patients, but may have side effects, such as lipodystrophic syndrome. This article aims to identify the frequency of the lipodystrophic syndrome and its associated factors in patients with HIV using antiretroviral therapy. It involved a cross-sectional study with HIV patients, monitored on an outpatient basis. The syndrome was evaluated by the association of two parameters: peripheral weight loss through the lipodystrophy severity scale and central fat accumulation, measured by the hip waist ratio. Poisson regression analysis was performed to identify the associated variables. Of the 104 patients evaluated, 27.9% presented the syndrome. After adjustment, the female sex (PRadjusted = 2.16 CI95% 1.43-3.39), being overweight (PRadjusted = 2.23 CI95% 1.35-2.65) and a longer period of use of antiretrovirals (PRadjusted = 1.64 CI95% 1.16-2.78), remained positively associated with the syndrome. On the other hand, a negative association with CD4 count £ 350 (PRadjusted = 0.39 CI95% 0.10-0.97) was observed The high prevalence of the syndrome and its association with specific groups reinforce the need for adequate follow-up and early identification to intervene in modifiable factors.

SUMMARY OBJECTIVES Individuals living with HIV seem to be more prone to changes in the redistribution of body fat, characterized as lipodystrophy, which may occur in conjunction with metabolic diseases. In the present study, such impacts were assessed in adults with and without HIV and associated with the time of virus diagnosis and treatment with antiretroviral. METHODS A cross-sectional study with 123 adults, in which 87 had HIV and 36 without HIV, of both sexes, in outpatient follow-up at the Specialized Care Service (SAE) in Macaé-RJ. The following were made: 1) Alteration in body fat distribution, measured by anthropometric parameters and self-reported lipodystrophy; 2) Biochemical profile; 3) Association between HIV diagnosis time and antiretroviral treatment. RESULTS 54.47% (n = 67) males, 45.52% (n = 56) females, mean age 37 years. Of these 87 were people living with HIV, 29% (n = 25) had self-reported lipodystrophy, mean time of virus infection, and antiretroviral treatment (5.80 ± 4.56 and 5.14 ± 3.82 years), respectively. Patients with self-reported lipodystrophy had a greater change in body fat distribution between 3-6 years of HIV diagnosis and a negative cholesterol profile. The antiretroviral treatment time influenced total cholesterol and triglycerides, even for patients without self-reported lipodystrophy, with a further nine years under treatment. CONCLUSION In this study, the negative cholesterol profile was mainly related to antiretroviral treatment time, even for patients without self-reported lipodystrophy, and changes in body fat distribution, measured by anthropometry, was especially associated with time for HIV infection in those with lipodystrophy self-reported.


RESUMO OBJETIVOS Indivíduos vivendo com HIV parecem mais propensos às alterações na redistribuição da gordura corporal, caracterizada como lipodistrofia, podendo acontecer em conjunto com as metabólicas. No presente estudo avaliaram-se tais impactos em adultos com e sem HIV e se associou ao tempo de diagnóstico do vírus e tratamento com antirretroviral. MÉTODOS Estudo tipo transversal, com 123 adultos, no qual 87 tinham HIV e 36 sem HIV, de ambos os sexos, em seguimento ambulatorial no Serviço de Atendimento Especializado (SAE) em Macaé - RJ. Foram feitos: 1) Alteração na distribuição da gordura corporal, mensurados por parâmetros antropométricos e lipodistrofia autorreferida; 2) Perfil bioquímico; 3) Associação entre tempo diagnóstico do HIV e tratamento com antirretroviral. RESULTADOS Incluíram-se 54,47% (n=67) do sexo masculino, 45,52% (n=56) do feminino, com média de idade de 37 anos. Destes, 87 eram pessoas vivendo com HIV, 29% (n=25) possuíam lipodistrofia autorreferida; tempo médio de infecção pelo vírus e tratamento antirretroviral (5,80±4,56 e 5,14±3,82 anos), respectivamente. Os pacientes com lipodistrofia autorreferida tiveram maior alteração na distribuição da gordura corporal entre 3-6 anos de diagnóstico do HIV e um perfil colesterolêmico negativo. O tempo de tratamento com antirretroviral influenciou o colesterol total e os triglicerídeos, mesmo para os pacientes sem lipodistrofia autorreferida, com mais de nove anos sob tratamento. CONCLUSÃO Neste estudo, o perfil colesterolêmico negativo se relacionou principalmente ao tempo de tratamento com antirretroviral, mesmo para os pacientes sem lipodistrofia autorreferida e as alterações na distribuição da gordura corporal, mensuradas por antropometria, se associaram especialmente ao tempo de infecção pelo HIV naqueles com lipodistrofia autorreferida.

ABSTRACT

O estudo teve como objetivo geral verificar o efeito da lipodistrofia associada ao HIV na autoestima e adesão à Terapia Antirretroviral (TARV) em pessoas vivendo com HIV (PVHIV). Trata-se de um estudo transversal, comparativo, descritivo e quantitativo, realizado no ambulatório de infectologia do Hospital São José de Doenças Infecciosas (HSJDI), em Fortaleza, Ceará. A amostra foi calculada para proporcionar um poder de 80% (ß=0,20) para detectar, ao nível de significância de 5%, diferenças clinicamente relevantes entre estratos de pacientes com e sem lipodistrofia, em relação à prevalência de baixa autoestima, sendo 125 participantes por grupo, totalizando 250 pacientes. Para amostragem, utilizou-se a técnica não probabilística por conveniência. Os critérios de inclusão foram: PVHIV de ambos os sexos; com 18 anos ou mais; em TARV por período igual ou superior a 12 meses. Foram critérios de exclusão: doença mental que impossibilitasse responder aos formulários; gestantes; moradores de rua e privados de liberdade em penitenciárias. A coleta de dados ocorreu de junho de 2018 a junho de 2019. Dados foram obtidos por meio de entrevista em local privativo, com duração média de 30 minutos e aplicação dos instrumentos: Formulário Sociodemográfico, Clínico e Epidemiológico para PVHIV; Escala de Autoestima de Rosenberg (EAR); Questionário para Avaliação da Adesão ao Tratamento Antirretroviral (CEAT-VIH). Para análise estatística, utilizou-se o software IBM SPSS Statistics versão 23.0. Variáveis quantitativas foram analisadas pelo teste de Shapiro-Wilk, verificando a normalidade da distribuição. Estratos de PVHIV com e sem lipodistrofia foram comparados quanto às características clínicas, sociodemográficas e epidemiológicas. Realizaram-se análise de regressão logística univariada e multivariada para verificar associações da autoestima e adesão à TARV com os fatores clínicos, sociodemográficos e epidemiológicos. A força de tal associação foi mensurada pela razão de chances (RC) bruta, sua precisão (intervalo de confiança de 95%) e significância (teste de Wald) da estimativa. Nas análises, empregaram-se testes bicaudais, considerando-se estatisticamente significante P<0,05. O projeto foi aprovado pelos Comitês de Ética da Universidade Federal do Ceará (N° 2.481.618) e HSJDI (N° 2.511.980). Foram seguidas as diretrizes do Strengthening the Reporting of Observational Studies in Epidemiology (STROBE). Da amostra total, 57,2% tiveram autoestima insatisfatória e 57,6% adesão adequada à TARV. A autoestima foi mais baixa em PVHIV com lipodistrofia (66,4%). Na análise univariada, a renda mensal menor ou igual a dois salários mínimos relacionou-se com autoestima insatisfatória (RC: 2,94; P<0,001), e PVHIV com lipodistrofia tiveram menor autoestima (RC: 2,14; P=0,003). Quanto à adesão à TARV, católicos tiveram melhor adesão (RC: 2,50; P=0,014), e renda mensal menor ou igual a dois salários mínimos vinculou-se a menor adesão (RC: 2,30; P=0,004). PVHIV com autoestima insatisfatória tiveram adesão mais inadequada à TARV (RC: 3,66; P<0,001). Estas associações confirmaram-se na análise multivariada, na qual PVHIV com renda mensal menor ou igual a dois salários mínimos (RC: 4,12; P<0,001) e aqueles com lipodistrofia tiveram autoestima mais insatisfatória (RC: 3,05; P<0,001). Os católicos tiveram melhor autoestima (RC: 2,94; P=0,012), quando comparados aos sem religião. Na adesão à TARV, pacientes com renda mensal inferior ou igual a dois salários mínimos (RC: 2,15; P=0,021) e pessoas com autoestima insatisfatória apresentaram adesão mais inadequada (RC: 2,68; P=0,001). Os católicos aderiram melhor aos antirretrovirais (RC: 3,13; P=0,007). Em conclusão, a lipodistrofia e a baixa renda interferiram negativamente na autoestima das PVHIV. A baixa renda e a autoestima insatisfatória interferiram de forma negativa na adesão à TARV. Porém, a religião foi um fator protetor para a autoestima satisfatória e adesão adequada aos antirretrovirais. (AU)

ABSTRACT

In the current era of effective antiretroviral therapies (ARTs), human immunodeficiency virus (HIV) infection became a chronic disorder that requires long term follow-up. Among other medical issues, these patients may develop endocrine problems, specific to HIV infection and its treatment. The purpose of this review is to give an overview of common endocrine complications associated with HIV infection, and to propose diagnostic and therapeutic strategies. HIV can affect the endocrine system at several levels. Adrenal and gonadal dysfunction, osteoporosis with increased fracture risk, dyslipidemia with increased cardiovascular risk, are some of the endocrine disorders prevalent in HIV-infected patients that may negatively influence quality of life, and increase morbidity and mortality. While ARTs have dramatically increased life expectancy in the HIV-infected population, they are not devoid of adverse effects, including endocrine dysfunction. Physicians caring for HIV-infected patients should be knowledgeable and exercise a high index of suspicion for the diagnosis of endocrine abnormalities, and in particular be aware of those that can be life threatening. Endocrine evaluation should follow the same strategies as in the general population, including prevention, early detection, and treatment.

Objetivo: descrever o perfil de adesão à terapia antirretroviral (TARV) de pacientes com Síndrome Lipodistrófica do Vírus da Imunodeficiência Humana (SLHIV), em uma unidade especializada do Estado do Pará, Brasil. Método: estudo qualiquantitativo, envolvendo questionário e prontuários de 124 pacientes, no período de fevereiro e março de 2013, após aprovação por Comitê de Ética. Na análise, a variância (p<0,05) articulou testes estatísticos, com dados apresentados em tabelas. Resultados: as dificuldades de entender e conhecer o esquema terapêutico, o Diabetes Mellitus e as alterações da lipodistrofia foram significantes na interferência da adesão à TARV. O grupo de adesão irregular está em risco para a eficácia do tratamento. Os demais têm a possibilidade de migração de um grupo para outro a qualquer momento. Conclusão: o perfil de adesão à TARV não é definida por dados socioeconômicos. A participação em grupos de adesão deve ser estimulada como fator de reversão do padrão de abandono. Objective: to profile adherence to antiretroviral therapy (ART) by patients with human immunodeficiency virus lipodystrophy syndrome (HIVLS) at a specialized unit in Pará, Brazil. Method: this quali-quantitative study, involving a questionnaire and medical records of 124 patients, was conducted from February to March 2013, after research ethics approval. In the analysis, statistical tests were related by variance (p <0.05) and data were presented in table form. Results: the difficulties of understanding and knowing the therapeutic regimen, mellitus diabetes and changes in lipodystrophy were significant in the interference of ART adherence. The irregular adherence group is at risk for treatment efficacy. The others have the possibility of migrating from one group to another at any time. Conclusion: the profile of adherence to ART is not defined by socioeconomic data. Participation in membership groups should be encouraged as a factor in reversing the pattern of abandonment.


Objetivo: describir el perfil de adhesión a la terapia antirretroviral (TARV) de pacientes con síndrome lipodistrófico del virus de la inmunodeficiencia humana (SLHIV), en una unidad especializada del Estado de Pará, Brasil. Método: estudio cualiquantitativo involucrando cuestionario e historias clínicas de 124 pacientes, en el período de febrero y marzo de 2013, después de la aprobación por el Comité de Ética. En el análisis, la varianza (p <0,05) articuló pruebas estadísticas, con datos presentados en tablas. Resultados: las dificultades en entender y conocer el esquema terapéutico, la Diabetes Mellitus y las alteraciones de la lipodistrofia fueron significantes en la interferencia de la adhesión a la TARV. El grupo de adhesión irregular está en riesgo para la eficacia del tratamiento. Los demás tienen la posibilidad de migración de un grupo a otro en cualquier momento. Conclusión: el perfil de adhesión a la TARV no está definido por datos socioeconómicos. La participación en grupos de adhesión debe ser estimulada como factor de reversión del patrón de abandono.

ABSTRACT

INTRODUÇÃO: Desde a introdução do tratamento do HIV com uso da terapia antirretroviral altamente ativa, a mortalidade por essa doença foi reduzida drasticamente em todo o mundo. Um dos parefeitos relacionados à utilização desses fármacos é a lipodistrofia glútea. O objetivo deste trabalho é verificar o impacto da correção dessa deformidade na qualidade de vida de pacientes com HIV. MÉTODOS: Foi conduzido um estudo de coorte histórica com 23 pacientes submetidos à gluteoplastia com implante intramuscular, entre janeiro de 2010 e dezembro de 2014, avaliando a qualidade de vida por meio do em Nottingham Health Profile em. As informações foram coletadas de julho a agosto de 2015. A análise estatística foi feita utilizando-se o em Related-Samples McNemar Test em. RESULTADOS: strong Houve diferença significativa entre o pré-operatório e pós-operatório em 19 das 38 perguntas. CONCLUSÃO: É possível afirmar que a reconstrução glútea melhora a qualidade de vida de pacientes HIV positivos acometidos por lipodistrofia glútea relacionada a antirretrovirais.


INTRODUCTION: Since the introduction of highly active antiretroviral therapy for the treatment of human immunodeficiency virus (HIV), disease mortality has been dramatically reduced worldwide. One related side effect from the use of these drugs is gluteal lipodystrophy. The aim of this study is to assess the quality-of-life impact of correcting this deformity in HIV patients. METHODS: A historical cohort study was conducted between January 2010 and December 2014 with 23 patients, assessing the quality of their lives using the Nottingham Health Profile. A statistical analysis was performed using the McNemar test for related samples. RESULTS: There was a significant difference between preoperative and postoperative response in 19 of the 38 questions. CONCLUSION: We may say that gluteal reconstruction plays a role in improving quality of life for HIV patients who have been affected by antiretroviral related gluteal lipodystrophy.

ABSTRACT

A AIDS caracteriza-se como doença degenerativa, crônica e progressiva. Após a introdução da terapia antirretroviral (TARV), observou-se uma importante redução na morbimortalidade. No entanto, ao longo do tratamento, os indivíduos podem apresentar alterações cardiometabólicas. Objetivo: Avaliar o perfil clínico-nutricional de portadores de HIV atendidos em um hospital de referência do Nordeste brasileiro. Métodos: Estudo do tipo descritivo transversal realizado no ambulatório e na unidade de internação de Doenças Infecto-Parasitárias (DIP) do Hospital das Clínicas da Universidade Federal de Pernambuco. Foram avaliadas variáveis nutricionais e clínicas. A amostra foi não probabilística por conveniência. Os dados foram digitados no programa Excel para Windows® e as análises realizadas no Programa SPSS® versão 13.0, com nível de significância de 5% para rejeição de hipótese de nulidade. Resultados: Foram avaliados 110 pacientes (54,5% em enfermaria). A média de idade foi 39±11 anos, e 15,4% possuíam 50 anos ou mais. Do total, 64,5% eram do sexo masculino. Entre os pacientes de enfermaria, 52,5% apresentaram complicações metabólicas; 1,6%, lipodistrofia e 15%, síndrome consumptiva, enquanto os pacientes ambulatoriais apresentaram valores de 76%, 4% e 4%, respectivamente. Tempo de diagnóstico, indicação, tempo e adesão à TARV não mostraram diferenças entre os grupos. Conclusão: As alterações metabólicas relacionadas a risco cardiovascular e à preservação do estado nutricional prevaleceram nos pacientes ambulatoriais, enquanto o perfil clínico-nutricional dos internados mostrou a desnutrição energético-proteica como a complicação mais frequente, com maior comprometimento do sistema imunológico e maior frequência de infecções oportunistas e sintomas gastrointestinais. O uso de drogas lícitas e/ou ilícitas, a falta de conhecimento sobre a importância da medicação e a negligência no autocuidado foram os principais motivos para o uso irregular da TARV


The acquired immunodeficiency syndrome is a chronic, progressive, and degenerative disease. Since the introduction of antiretroviral therapy (ART), a significant reduction in mortality and morbidity rates has been recorded. However, patients may present cardiometabolic changes throughout the treatment. Objective: To assess the clinical and nutritional profile of HIV patients treated in a referral hospital in northeast Brazil. Methods: Descriptive cross-sectional study performed in inpatient and outpatient units for infectious and parasitic diseases at the Hospital of the Universidade Federal de Pernambuco Nutritional and clinical variables were assessed. The sample was a non-probabilistic convenience sample. The data were entered into Excel for Windows® and analyzed using SPSS® software (version 13.0), with a 5% level of significance set for the rejection of the null hypothesis. Results: In total, 110 patients were assessed (54.5% in infirmary). The mean age was 39±11 years, with15.4% of the individuals aged 50 years or more. Of the total, 64.5% were male. Among the patients in the ward, 52.5% had metabolic complications; 1.6% had lipodystrophy, and 15% had wasting syndrome, while ambulatory patients showed values of 76%, 4%, and 4%, respectively. Time of diagnosis, indication, time, and adherence to ART showed no differences between the groups. Conclusion: The metabolic changes related to cardiovascular risk factors and the preservation of nutritional status prevailed in ambulatory patients, while the clinical-nutritional profile of hospitalized patients showed energy-protein malnutrition as the most common complication, with greater involvement of the immune system and increased frequency of opportunistic infections and gastrointestinal symptoms. The use of licit and/or illicit drugs, the lack of knowledge about the importance of medication, and the negligence in care were the main reasons for the irregular use of ART.

ABSTRACT

O objetivo do estudo foi verificar o efeito de 12 semanas de treinamento físico resistido periodizado sobre os perfis glicêmico, lipídico, imunitário, antropométrico e força muscular em um paciente vivendo com HIV e lipodistrófico. O indivíduo do estudo era do sexo masculino, 46 anos, etnia branca, diagnosticado há 17 anos com HIV e há 3 anos com lipodistrofia; o mesmo foi submetido a 12 semanas de treinamento físico resistido periodizado de forma ondulatória diária (TRPO), três vezes por semana. Antes e após este período foram realizadas avaliações antropométrica, bioquímica, imunitárias e de força muscular. Após TRPO, houve diminuição das circunferências de pescoço, peito, abdômen, percentual de gordura, massa gorda, bem como de gorduras subcutânea total, central e periférica. Concomitantemente houve aumento das circunferências de quadril, braços, coxas e panturrilhas, além do aumento de massa muscular. Houve diminuição nos níveis de glicemia de jejum, triglicerídeos, colesterol total, LDL-colesterol, HB1AC, monócitos e aumento do HDL-colesterol, leucócitos, linfócitos, neutrófilos, monócitos e plaquetas. Como conclusão demonstramos que 12 semanas de TRPO foi benéfico e seguro no tratamento das desordens metabólicas e antropométricas em um paciente vivendo com HIV lipodistrófico.(AU)


The purpose of the study was to evaluate the effect of 12 weeks of resistance exercise training periodization on the glycemic, lipid, immune, muscular strength and anthropometric profiles in a patient living with HIV and lipodystrophy. The subject of the study was male, 46 years old, white ethnicity, diagnosed 17 years ago with HIV and 3 years with lipodystrophy; the same was submitted to 12 weeks of daily undulating resistance training (DUPRT) three times per week. Before and after this period were conducted anthropometric, biochemical, immune and muscle strength. After DUPRT, there was reduction in neck, chest and abdomen circumferences, fat percentage, fat mass and total, central and peripheral subcutaneous fat. Concurrently there was an increase of hip, arms, thighs and calves circumferences, and increased muscle mass. There was a decrease in fasting glucose levels, triglycerides, total cholesterol, LDL-cholesterol, Hb1Ac, monocytes and increased HDL-cholesterol, white blood cells, lymphocytes, neutrophils, monocytes and platelets. In conclusion we demonstrated that 12 weeks of TRPO was beneficial and safe in the treatment of metabolic and anthropometric disorders in a patient living with HIV and lipodystrophy.(AU)

Introduction: Highly Active Antiretroviral Therapy (HAART) has improved and extended the lives of thousands of people living with HIV/AIDS around the world. However, this treatment can lead to the development of adverse reactions such as lipoatrophy/lipohypertrophy syndrome (LLS) and its associated risks. Objective: This study was designed to assess the prevalence of self-reported lipodystrophy and nutritional status by anthropometric measurements in patients with HIV/AIDS. Methods: An observational study of 227 adult patients in the Secondary Immunodeficiencies Outpatient Department of Dermatology, Hospital das Clínicas, Faculty of Medicine, University of São Paulo (3002 ADEE-HCFMUSP). The sample was divided into three groups; Group 1 = 92 patients on HAART and with self-reported lipodystrophy, Group 2 = 70 patients on HAART without self-reported lipodystrophy and Group 3 = 65 patients not taking HAART. The nutritional status of individuals in the study sample was determined by body mass index (BMI) and percentage of body fat (% BF). The cardiovascular risk and diseases associated with abdominal obesity were determined by waist/hip ratio (WHR) and waist circumference (WC). Results: The prevalence of self-reported lipoatrophy/lipohypertrophy syndrome was 33% among women and 59% among men. Anthropometry showed depletion of fat mass in the evaluation of the triceps (TSF) in the treatment groups with HAART and was statistically independent of gender; for men p = 0.001, and for women p = 0.007. Similar results were found in the measurement of skin folds of the upper and lower body (p = 0.001 and p = 0.003 respectively). In assessing the nutritional status of groups by BMI and % BF, excess weight and body fat were more prevalent among women compared to men (p = 0.726). The WHR and WC revealed risks for cardiovascular and other diseases associated with abdominal obesity for women on HAART and with self-reported LLS (p = 0.005) and (p = 0.011). ...


Objetivos: A terapia antirretroviral altamente ativa (HAART) tem melhorado e aumentado a vida de milhares de pessoas que vivem com a infecção pelo HIV/AIDS em todo o mundo. No entanto, este tratamento pode levar ao desenvolvimento da síndrome da lipodistrofia (LDS). Este estudo foi desenvolvido para avaliar a prevalência de auto-relato de LDS, perfil nutricional e medidas antropométricas de pacientes com HIV/AIDS. Métodos: Estudo observacional de 227 pacientes adultos, divididos em: Grupo 1: 92 pacientes em HAART e com LDS; Grupo 2: 70 pacientes em tratamento com HAART e sem LDS e Grupo 3: 65 pacientes que não tomam HAART. O estado nutricional foi avaliado pelo índice de massa corporal (IMC) e o percentual de gordura corporal (%GC) por meio de medidas antropométricas. Resultados: A prevalência de auto-relato de LDS foi de 44% entre as mulheres e 39% entre os homens. DC do tríceps (PCT) apresentou-se mais elevada no grupo HAART e LDS (homens p < 0,001; mulheres p < 0,007) em comparação com aqueles sem HAART, respectivamente. IMC revelou excesso de peso para a maioria dos indivíduos. Conclusões: As medidas antropométricas foram úteis para confirmar a prevalência de auto-relato da síndrome da lipodistrofia. A avaliação das dobras dos braços e pernas revelou-se um bom método para avaliação antropométrica de lipoatrofia de membro, independentemente do sexo. Estes resultados permitiram o estabelecimento de estratégias para o diagnóstico precoce da LDS na prática clínica, em pessoas vivendo com HIV / AIDS. .

Introdução: O tratamento de pacientes portadores da síndrome da imunodeficiência adquirida deve ser integral e se basear no controle da doença e das complicações relacionadas ao uso de medicações antirretrovirais, como a lipodistrofia. Esse estudo tem como objetivo avaliar as principais queixas, os aspectos epidemiológicos e os procedimentos cirúrgicos realizados para corrigir a lipodistrofia em pacientes em uso crônico de antirretrovirais. Método: Estudo retrospectivo, no qual foram coletados dados dos prontuários de 27 pacientes submetidos a 36 procedimentos cirúrgicos relacionados à correção de lipodistrofia no período de março de 2010 a junho de 2014 no serviço de Cirurgia Plástica do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto. Resultados: A idade média dos pacientes foi 47,2 anos, 22,2% homens e 77,8% mulheres. O tempo médio de uso da terapia antirretroviral (TARV) foi de 12,1 anos. As queixas mais encontradas foram: giba dorsal (44,4%), lipodistrofia abdominal (44,4%) e lipoatrofia glútea (37,04%). Na maioria dos pacientes (70,4%), foi realizada uma cirurgia. Quanto às cirurgias, a lipoaspiração de giba foi realizada em 48,1% dos pacientes, seguida da lipoaspiração de abdome, dorso ou flancos (44,4%) e gluteoplastia (22,2%). Entre todos os 36 procedimentos realizados, apenas dois apresentaram complicações. O tempo médio de seguimento pós-operatório foi de 11,2 meses. Do total, 70,4% dos pacientes mostraram-se satisfeitos após os procedimentos. Conclusões: O sucesso do tratamento cirúrgico da lipodistrofia causada pelo uso da TARV baseia-se na seleção pré-operatória adequada e em seguimento constante e prolongado. A melhoria da autoestima facilita a adesão ao tratamento com antirretrovirais.


Introduction: Treatment of patients with acquired immunodeficiency syndrome should be complete and based on controlling the disease and the complications related to the use of antiretroviral medications, such as lipodystrophy. This study aimed to evaluate the main complaints, epidemiological aspects, and surgical procedures performed for lipodystrophy correction among patients receiving long-term antiretroviral therapy. Method: In this retrospective study, data were collected from the medical records of 27 patients who underwent 36 surgical procedures associated with lipodystrophy correction, from March 2010 to June 2014, at the Plastic Surgery Service of the Hospital das Clínicas, Faculty of Medicine of Ribeirão Preto. Results: The average age of the patients was 47.2 years; 22.2% were men and 77.8% were women. The average duration of antiretroviral therapy (HAAR ) was 12.1 years. The most frequent complaints were dorsal hump (44.4%), abdominal lipodystrophy (44.4%), and gluteal lipoatrophy (37.04%). The majority of patients (70.4%) had undergone surgery . The most common type of surgery performed was hump liposuction (carried out in 48.1% of the patients), followed by abdominal, back, or flank liposuction (44.4%) and gluteoplasty (22.2%). Among all 36 procedures performed, only 2 resulted in complications. The average postoperative follow-up period was 11.2 months. In total, 70.4% of patients were satisfied with the results of their procedure. Conclusions: The success of surgical treatment of HAARinduced lipodystrophy is based on proper preoperative selection as well as constant and prolonged follow-up. Improved selfesteem facilitates the adherence to antiretroviral drug treatment.

ABSTRACT

The aim of present study was to describe the frequency of lipodystrophy syndrome associated with HIV (LSHIV) and factors associated with dyslipidemia in Brazilian HIV infected children. HIV infected children on antiretroviral treatment were evaluated (nutritional assessment, physical examination, and laboratory tests) in this cross-sectional study. Univariate analysis was performed using Mann-Whitney test or Fisher's exact test followed by logistic regression analysis. Presence of dyslipidemia (fasting cholesterol >200 mg/dl or triglycerides >130 mg/dl) was the dependent variable. 90 children were enrolled. The mean age was 10.6 years (3-16 years), and 52 (58%) were female. LSHIV was detected in 46 children (51%). Factors independently associated with dyslipidemia were: low intake of vegetables/fruits (OR = 3.47, 95%CI = 1.04-11.55), current use of lopinavir/ritonavir (OR = 2.91, 95%CI = 1.11-7.67). In conclusion, LSHIV was frequently observed; inadequate dietary intake of sugars and fats, as well as current use of lopinavir/ritonavir was associated with dyslipidemia.

Introduction: The published literature shows an increased occurrence of adverse events, such as human immunodeficiency virus (HIV)-associated lipodystrophy syndrome, that are associated with the continuous use of antiretroviral therapy. This study was performed to estimate the prevalence and factors associated with lipodystrophy in acquired immune deficiency syndrome (AIDS) patients. Methods: We conducted a cross-sectional study between October 2012 and February 2013. The sample consisted of patients with AIDS who attended the Outpatient Treatment Center for Infectious Diseases at Nereu Ramos Hospital, Florianópolis, State of Santa Catarina, Brazil. We collected information on demographics, lifestyle, HIV infection, and clinical aspects of the disease. Self-reported signs of lipodystrophy and body measurements were used for lipodystrophy diagnosis. Results: We studied 74 patients (mean age 44.3±9.2 years; 60.8% men). Among the patients, 45.9% were smokers, 31.1% consumed alcoholic beverages, and 55.4% were sedentary. The prevalence of lipodystrophy was 32.4%, and sedentary subjects had a higher prevalence of lipodystrophy compared with physically active individuals. Conclusions: The prevalence of lipodystrophy was 32.4%. Physical activity was considered an independent protective factor against the onset of HIV-associated lipodystrophy. .

ABSTRACT

Objective: To discuss the relationship between the presence of lipodystrophy in patients with HIV / AIDS and outline actions that optimize quality of life better. Method: This is a research type of qualitative literature. Data collection was conducted during the months of July and August 2011, through the database of the Electronic Library of Brazilian Scientific Journals (SciELO Brazil) and pubmed. We use the following keywords for search: HIV / AIDS and Lipodystrophy. Results: Based on reading the articles grouped the main changes related to lipodystrophy in three categories: 1) psychosocial changes, 2) bodily changes, and 3) metabolic disorders. Conclusion: Given the complexity of the effects caused by lipodystrophy, highlights the importance of multidisciplinary team with these clients and their families seeking comprehensive care and promote quality of life.

Utilizamos os seguintes descritores para a busca: HIV/AIDS e Lipodistrofia. Resultados: Baseado na leitura dos artigos agrupamos as principais alterações relacionadas com a lipodistrofia em três categorias: 1)alterações psicossociais; 2) alterações corporais; e 3) alterações metabólicas. Conclusão: Tendo em vista a complexidade das repercussões ocasionadas pela lipodistrofia, destaca-se a importância do trabalho da equipe multiprofissional com essa clientela e seus familiares, buscando a integralidade da assistência e promoção da qualidade de vida.

Resultados: A partir de la lectura de los artículos agrupados los principales cambios relacionados con la lipodistrofia en tres categorías: 1) los cambios psicosociales, 2) los cambios corporales, y 3) trastornos metabólicos. Conclusión: Dada la complejidad de los efectos causados por la lipodistrofia, pone de relieve la importancia del equipo multidisciplinario con estos clientes y sus familias en busca de una atención integral y promover la calidad de vida.

BACKGROUND: HIV/AIDS-Associated Lipodystrophy Syndrome includes changes in body fat distribution, with or without metabolic changes. The loss of fat from the face, called facial lipoatrophy, is one of the most stigmatizing signs of the syndrome. OBJECTIVES: To evaluate the effect of FL treatment using polymethylmethacrylate (PMMA) implants on disease progression, assessed by viral load and CD4 cell count. METHODS: This was a prospective study of 44 patients treated from July 2009 to December 2010. Male and female patients, aged over 18 years, with clinically detectable FL and who had never been treated were included in the study. PMMA implantation was done to fill atrophic areas. Laboratory tests were conducted to measure viral load and CD4 count before and after treatment. RESULTS: Of the 44 patients, 72.72% were male and 27.27% female, mean age of 44.38 years. Before treatment, 82% of patients had undetectable viral load, which increased to 88.6% after treatment, but without statistical significance (p = 0.67). CD4 count before treatment ranged from 209 to 1293, averaging 493.97. After treatment, the average increased to 548.61. The increase in CD4 count after treatment was statistically significant with p = 0.02. CONCLUSION: The treatment of FL with PMMA implants showed a statistically significant increase in CD4 count after treatment, revealing the impact of FL treatment on disease progression. FUNDAMENTOS: A Síndrome Lipodistrófica Associada ao HIV/AIDS compreende alterações na distribuição da gordura corporal, acompanhada ou não de alterações metabólicas. A perda da gordura facial, chamada lipoatrofia facial, é dos sinais mais estigmatizantes da síndrome. OBJETIVOS: Avaliar o impacto do tratamento dalipoatrofia facial com implante de polimetilmetacrilato sobre a progressão da doença, avaliada pela contagem da carga viral e a contagem de células CD4. MÉTODOS: Estudo prospectivlipoatrofia facial clinicamente detectável e sem tratamento prévio. Foi realizado implante de polimetilmetacrilato para preenchimento das áreas atróficas. Foram realizadas contagem da carga viral e células CD4 antes e após o tratamento. RESULTADOS: Dos 44 pacientes, 72,72% eram do sexo masculino e 27,27% do feminino, e idade média de 44,38 anos. Antes do tratamento, 82% dos pacientes apresentavam carga viral indetectável, que aumentou para 88,6% após o tratamento, mas sem significância estatística (p=0,67). A contagem de CD4 antes do implante variou de 209 a 1293, com média de 493,97. Após o tratamento, essa média aumentou para 548,61. ABSTRACT

OBJECTIVE: To study whether patients with HIV-1 associated lipodystrophy (LD) on highly active antiretroviral treatment (HAART) have more psychopathology and worse psychosocial adjustment than a similar group without this syndrome. METHODS: In a cross-sectional, observational study we compared 47 HIV-1 infected patients with LD (LD group) with 39 HIV-1 infected patients without LD (non-LD group). All participants were on HAART. The Beck Depression Inventory (BDI), the State and Trait Anxiety Inventory (STAI) and the Goldberg Health Questionnaire (GHQ-60) were administered. Levels of familial, work and social adjustment and adjustment to stressful events were evaluated in a semi-structured interview. Clinical information was extracted from the clinical records. RESULTS: In the univariate analysis patients with LD showed higher state anxiety scores (p = 0.009) and worse work adjustment (p = 0.019) than those without LD. A total of 45.3% of LD patients scored above the cut-off point on the trait anxiety scale, and over 33.3% scored above the cut-off point on the BDI, GHQ and state anxiety scales. However, in multivariate analyses LD was not independently associated with psychopathology or with worse adjustment in the studied areas. CONCLUSIONS: The finding that LD was not a predictor of greater psychopathology or worse psychosocial adjustment in HIV-1 infected patients, despite the high scores found, suggests that factors not taken into account in this study, such as LD severity and self-perception should have been included in the analysis. Further studies including a greater number of variables and a larger sample size will advance our understanding of this complex condition.

ABSTRACT

O presente estudo investigou o efeito de 24 semanas de treinamento resistido (TR) sobre a força e hipertrofia muscular de pacientes HIV-soropositivos. Participaram deste estudo 45 voluntários submetidos à terapia antirretroviral fortemente ativa (HAART), destes, 23 realizaram 3 sessões semanais, com 10 repetições a 80% 1RM. O teste de 1RM foi realizado de acordo com a metodologia proposta por Kraemer e Fry (1995), para estimativa da hipertrofia muscular adotou-se as equações de Frisancho (1984). Em comparação aos valores, o TR melhorou a força de 1RM nos exercícios de agachamento em 49% (21,0±4,9 vs. 31,2±5,1; P=0,001), supino reto em 13% (34,3±8,1 vs. 39,8±9,4; P=0,04), cadeira extensora em 34,1% (26,3±7,1 vs. 37,1±6,6; P=0,01), tríceps em 51% (22,9±4,0 vs. 38,3±4,9; P=0,001), pulley costas em 31,5% (31,7±3,9 vs. 41,7±4,4; P=0,01), cadeira flexora em 37,2% (18,9±3,4 vs. 27,3±3,2; P=0,01) e rosca bíceps em 60% (27,9±6,9 vs. 40,4±4,5; P=0,001). Não foram observadas diferenças (P<0,05) entre os valores basais e finais para o grupo controle. Observou-se aumento significativo (P<0,05) na área muscular do braço isenta de massa óssea, no grupo TR (52,8±14,5 cm²) em relação ao controle (39,5±12,4 cm²). Ademais, o TR resultou em significativa (P<0,05) redução da glicemia sanguínea de jejum (96,5±18,3 vs. 90,5±12,6), pressão arterial sistólica (126,3±14,3 vs. 120,0±10,0) e circunferência de cintura (83,0±12,5 vs. 80,6±10,2). Conclui-se que seis meses de TR resultaram em melhora na força e hipertrofia, ademais, o treinamento aplicado contribuiu para a regulação das variáveis metabólicas dos pacientes. Uma vez que a HAART é inevitável ao HIV-soropositivo, recomenda-se que o exercício físico seja realizado no intuito de dirimir os efeitos colaterais advindos desta terapia.


The aim of this study was to investigate the effect of 24 weeks of resistance training (RT) on the strength and muscle hypertrophy in patients with HIV-seropositive. Participated in the study 45 subjects undergoing highly active antiretroviral therapy (HAART). They were divided into two groups: control (n=22) and RT (n=23). The RT group realized three sessions, with 10 repetitions at 80% of 1MR. The MR tests were performed pre and post 24 weeks according to Kraemer and Fry (1995) and the equations proposed by Frisancho (1984) were adopted to estimate bone-free upper arm muscle area. Compared to baseline, the RT improved the strength of 1MR in the squat exercise at 49% (21.0±4.9 vs. 31.2±5.1; P=0.001), bench press by 13% (34.3±8.1 vs. 39.8±9.4, P=0.04), leg extension in 34.1% (26.3±7.1 vs. 37.1±6.6, P=0.01), triceps in 51% (22.9±4.0 vs. 38.3±4.9, P=0.001), pulley in 31.5% (31.7±3.9 vs. 41.7±4.4, P=0.01), leg curl in 37.2% (18.9±3.4 vs. 27.3±3.2, P=0.01) and biceps in 60% (27.9±6.9 vs. 40.4±4.5, P=0.001); there were no significant differences between baseline and final at control. The Bone-free upper arm muscle area at RT group (52.8±14.5 cm²) was significant increased (P<0.05) comparing to control (39.5±12.4 cm²). Moreover, the RT resulted in significant reduction (P<0.05) in fasting blood glucose (96.5±18.3 vs. 90.5±12.6, P<0.05), systolic blood pressure (126.3±14.3 mmHg vs. 120.0±10.0 mmHg) and waist circumference (83.0±12.5 cm vs. 80.6±10.2 cm). We conclude that six months of RT resulted in improvement in strength and hypertrophy; in addition, this training contributed to regulate the metabolic variables from these patients. Since the HAART is inevitable to HIV-seropositive, It's recommended that physical exercise be realized to minimize the side effects from this therapy.
